The iliotibial band is a thick fascial structure that begins at the pelvis and then runs along the outer aspect of the thigh to insert on the lateral aspect of the tibia. This structure crosses the lateral aspect of the hip (greater tuberosity) and a tight band can result in pain associated with an audible noise or visible POP. This is commonly referred to as the external snapping hip syndrome. Initial treatment is non-operative with rest, stretching, using a foam roller and physical therapy. If non-operative treatment is unsuccessful hip arthroscopy or open surgery can be performed to either lengthen or release the band.
