The trochanteric bursa is a fluid filled sac located adjacent to the edge of the upper femur. The most common cause of bursitis is inflammation as a result of local trauma or strain. Risk factors are repetitive activities such as stairclimbing, running and similar movements. The symptoms are mostly localized tenderness over the outer hip. Dull pain worsening with walking, exercise, running or stairclimbing is common. The bursitis often makes it difficult to lie on the involved side. An initial short course of anti-inflammatory medication and physical therapy is helpful. Cortisone injection is normally rapidly effective. If treatment does not resolve the problem, excision of the bursa either open or arthroscopic is quite effective. If the symptoms do not resolve an MRI should be performed to rule out tears in the gluteus medius tendon.
